A transition occurs when there is a change from one move to another move; changing from a knee lift to a jog is a transition.
Passage from one state or position to another, or from one part to another part; a change in health status, roles, family, abilities, and other important areas. Transitions often require adaptations within the person, the group, or the environment and define the need for and context of nursing care.
