An objective indication of disease discoverable by the clinician upon evaluation of the patient.

An observable skin rash or a measurable fever is a sign, for example, while the feeling of itching or heat is a symptom. If indications are clear-cut and obvious, they are called hard signs; if subtle and not so clear-cut, they are called soft signs, as in relation to diagnosing learning disabilities or schizophrenia. Among physicians, a sign can also be an observation that fairly conclusively indicates the presence of a particular condition, as Brudzinski’s sign indicates meningitis. Sign is also sometimes used as synonymous with reflex.
