    In epidemiology, an incident rate that extends over a given period of time and for which all subjects are followed for this same period of time.

    A form by school health service professionals to maintain records of screening tests, medical examinations, immunizations, illness or disabilities, anecdotal records of health-related behaviors that threaten a pupil’s learning ability, and any other relevant information with respect to the education and well-being of a child throughout his or her education.
