Community care plan

The Eutaw Group’s alternative to (or complement to) the accountable health plan (AHP) in the health care reform movement (1993). A CCP is expected to carry out a proactive “predict and manage” philosophy; its proponents see the AHP as essentially a “treat and/or prevent what comes in the door” organization. It is directed at “equal health status” rather than “equity in prices per capita,” as it describes the AHP’s goal. A CCP would offer (1) “essential gateway services,” accessible via a “gatekeeper mechanism ” in the hands of a generalist (physician or allied health professional, with physician oversight; see gatekeeper and (2) “essential referral services.”
