  • Geriatric consultation team

    An in-hospital team, typically consisting of a physician, a clinical nurse specialist, and a social worker, all specialized in geriatric medicine and the problems of the elderly. Such a team evaluates the needs of a geriatric patient on admission to the hospital and provides recommendations on the patient’s care to the direct care providers.
